Nellie Formanack was born on May 2, 1918 in Coppingford, England to James and Sarah (Speechley) Swales. She passed away on Sunday, February 24, 2008 at Community Memorial Hospital in Syracuse, NE at the age of 89 years, 9 months and 22 days.

A graduate of Peterbourgh District Hospital School of Nursing, she worked as a staff nurse until she was united in marriage to Dr. Carl J. Formanack in Peterbourgh, England on September 12, 1942. After the war, she moved to the United States where she made her home in Otoe County. She was a dedicated member of St. Paulinus Catholic Church and Altar Society.

She is survived by Sons: Tom Formanack and wife Gail of Omaha, Nebraska; Bill Formanack and wife Cathy of Talmage, NE; George Formanack and wife Dixie of Nebraska City, NE; Daughter: Ann Lerdahl and husband Al of Nebraska City, NE; 12 Grandchildren, 7 Great Grandchildren, Brother: William Swales and Sister: Thelma Davies both of England

She was preceded in Death by her parents, husband, Dr. Carl Formanack, sons, Mike & Jim Formanack, daughter-in-law: Sandy Formanack, brother, Jim Swales and sister Annie Swales

Funeral Services will be held on Friday, February 29, 2008 at 10:30 AM at St. Paulinus Catholic Church in Syracuse, NE with Father Michael Stec officiating.

Closed Casket visitation will be held on Wednesday, February 27th from 9 AM - 9 PM and Thursday, February 28th from 9 AM until 5:30 PM with a Rosary Service held at the Church at 7:30 PM

Burial will be in Park Hill Cemetery in Syracuse, NE.
